Monitoring of Care
With a change in health status
or living circumstances there may be a need to coordinate care
from a diverse group of individuals and businesses. Our Care Manager is involved with the client and family in
assessing their needs and then providing information and
referral for the best possible care and service. Services can
include:

Home Visits involve our
Care Manager visiting clients in their homes, whether
this be a private residence or institution. They review the
on-going care of the client and review the services being
supplied by other providers.
In the visit our care manager
makes observations about the client's medical, emotional, and
social situations. Information is maintained in a computerized
tracking program and periodic reports are generated for review
by those individuals identified by the client as needing to
know.
The care manager can
also be involved in providing help with mailing tasks,
organizing appointments, and coordinating other services such as
transportation, shopping and health appointments.
Telephone Monitoring
In providing comprehensive
care we believe it essential that the client know that help is
only a telephone call away. Our care manager is available 24
hours a day to answer questions. A routine telephone contact is
made at an interval agreed to by the client and this time is
used to offer reassurance and to offer guidance.
