Our "Life Assist" Services recognizes that the best care is achieved when a "total life" approach is followed. This approach views the relationship with our clients as on-going over time with care being assigned according to what is needed at that point in time.

      Operated as a separate service from our Home Health Agency our "Homestead At-Home" Care Manager has a much broader service plan that spans across the lifespan of our clients. Services may include assistance with medical, psychosocial, functional, financial, and other life planning needs which are needed by the client. Coordinating care for your loved ones can involve a wide range of service possibilities.

     We designed this service to provide management guidance in dealing with the many different issues that can arise in later life. Our Care Manager serves as a source of information and referral to other specialists who offer expertise in those areas. In addition the care manager acts in a monitoring manner to ensure that proper care is being provided by others involved in the loved ones care. Homestead At-Home is there to help guide the client and their family in locating resources and finding solutions. We have developed a directory of local providers and businesses that can lessen the anxiety of what to do. Available in both an on-line directory as well as a print edition that is given to the client the listing contains information about the provider or business and their offering. Homestead At-Home does not accept any reimbursement for the listing and each entity is an independent provider of service. Examples of listings in the directory include:
