
What is Homestead
At-Home?With the improvement in
health care Americans are living longer. Most older people and those who
face other health challenges prefer to remain in their homes as long as
possible. Family and friends may be able to provide resources to a degree so
that their loved ones can be where they want to be however the complexity of
health care, economic factors, and other obligations may overwhelm the best
intentions to help.
"Homestead At-Home" is a management service
that partners with individuals and their families so that greater
independence is achieved and the person is able to have their life. Designed
around a core group of Life Care Managers the service works from a plan of
action that may include health care services, rehabilitation, health
promotion and social service interventions.
Who Uses Our
Program
Anyone who is involved with
the care of an older adult may seek out our service. Our
Care Manager does not replace family involvement but
acts in a manner to help gather information for decisions and
carrying out action plans. Care managers may be hired by:
*Spouses
*Children or other Family Members who may live some
distance from
the client
*Guardians or conservators
*Attorneys
*Trust Directors

Services
Recognizing the broad nature
of life challenges our program offers three avenues of service.

Direct Service:
Our Care Manager may be
directly involved with the client or family. Services might
involve health or other assessment services of the person or
review of living circumstances or monitoring of care.

Cooperative Involvement:
Acting
on behalf of the client or family our Care Manager may
operate in a cooperative manner with others in ensuring care
and services. They may act on behalf of the client in
establishing services or in maintaining a network of
communication.

Information and Referral:
Our Care Manager maintains an active
directory of professionals and service providers who have
been identified as community resources. These individuals
and businesses are independent of "Homestead At-Home" and you can
be sure that our recommendations are not biased as we seek
the best for the client.
Service Payment
"Homestead At-Home" Care Management
services are based on a fee-for-service arrangement. We do not
receive payment from Medicare or Medicaid and we do not take
referral fees from any of the professionals that we may list in
our directories of providers that are given to our clients. We
do not accept such fees so that you know that we are unbiased in
our review of local providers and institutions selecting only
what is best for the client.

DAYTIME $17.50/hour
(8 a.m. to 5 p.m. 2 hour minimum)
12 HOUR OVERNIGHT $185.00
(with 8 hours sleep)
24-HOUR LIVE-IN $380.00
Bath & Grooming Service Only $15.00
(not to exceed one hour)
Weekly Medication Setup Nurse Visit $50.00
MILEAGE 40 cents/mile
Caregiver's care- in route to Client's home (applies if caregiver's vehicle
used)
HOLIDAY RATES
Time-and-a half for the following:
New Year's Eve/Day Easter
Memorial Day July 4th
Labor Day Thanksgiving
Christmas Eve/Day
Assessment
Services
Initial Assessment....
$550.00
Includes an initial in-home consultation with our
Care Manager, an inspection for environmental safety,
development of an emergency plan, organization of health
information and creation of record library, and any follow-up or
further investigation to complete the written report. We then
will meet with the client and family as desired to go over the
report and recommendations.
In-Office Assessment....
$200.00
Includes a 1.5 hour interview with the client by the
Care Manager held at our office. Information is
obtained completing a medical and social history so that client
needs can be identified. Specific information is provided
concerning what action is needed and what strategies are best
suited to obtain these goals.
Monthly
Maintenance... $35.00
Includes a monthly telephone monitoring call, access to
our directory of service providers, and assistance with
obtaining resources.
Other services are offered
on an "as needed" basis and the fees vary according to the nature of service
and personnel used.
